Program Overview
The self-determination program is a voluntary, alternative way to receive services and supports. Divine Inclusions will provide support which assist Individuals with their transition from traditional services into the Self-Determination Program (SDP). Services will consist of an individualized person- centered program plan, which includes assisting the Individual to identify and meet potential SDP needs under the guidelines of the department. Services will be provided in- office, in-home, and in the community.

Program Entry Requirments
Divine Inclusions will be available to assist Individuals of all ages and gender who has completed the self-determination orientation and is interested in transitioning from traditional services into the self-determination program.
Self-Determination Program (SDP) Entry Requirements
Individuals over the age of three (3) with Developmental Disabilities who receives regional center services, who lives with family or in a community setting. Individuals who agrees to participate in an orientation for the Self-Determination Program, work with a Financial Management Services (FMS) entity to manage the individual budget and agrees to only purchase services eligible for federal reimbursement and that are not available through a generic agency.
*Individuals who live in a licensed, Long-term Health Facility, such as a Skilled Nursing Facility, Developmental Center, or an Intermediate Care Facility are NOT eligible for SDP

Acupuncture, Behavioral Intervention, Chiropractic, Communication Support, Community Integration, Community Living, Dental, Employment, Environmental Accessibility Adaptations, Family Support, Family/Consumer Training, Financial Management, Home Health Aide, Homemaker, Housing Access, Independent Facilitator, Individual Training and Education, Lenses and Frames, Live-In-Care, Message Therapy, Non-Medical Transportation, Nutritional Consultation, Occupational Therapy, Optometric/Optician, Participant-Directed Goods, Personal Emergency Response Systems (PERS), Physical Therapy, Prevocational Supports, Psychology Services, Respite, Skilled Nursing, Specialized Medical Equipment, Technology, Training and Counseling, Transition/Set Up Expenses, Vehicle Modifications and Adaptations, and Speech, Hearing, and Language Services.
