Person-Centered Approach
Divine Inclusions will assist you in setting goals which aligns with the future you envision for yourself. Divine Inclusions will provide person-centered services which includes assisting, coaching, and/or training supports needed by a potential SDP participant and their family or their representative to successfully enroll into SDP. Divine Inclusion will assist individuals and their families with navigating through the SDP process increasing their ability to gain more freedom, control, and responsibility in choosing services and supports to meet their goals in life and their Individual Program Plan (IPP).
Person-Centered Plan
Divine Inclusion will support Individuals with creating a person-centered plan. The person-centered team is led by the Individual and includes the voice of their circle of support. The person-centered team is structured to ensure the Individual has a plan in place for a continual process of listening, learning, and actions that helps the Individual to secure a future of their dreams. Divine Inclusions will ensure the person-centered plan is committed to recognizing and supporting the rights of the Individual. Divine Inclusions will ensure the person-centered plan reflects the Individuals capacities, what’s important to and for, likes and dislike, and supports needed. Divine Inclusions is committed to a process-oriented approach empowering Individuals to plan their life, find their voice, work towards reaching their goals, and allowing Individuals to be the author and center of the planning process.

- Assist with understanding the transition from traditional services to SDP
- Assist with identifying available SDP services

- Coordination of Services and Supports
- Introduce interviewing techniques
- Assist with understanding the onboarding and off-boarding of staff and providers
- Assist with understanding negotiating rates for staff and providers
- Assist individuals and families on qualifications of providing STP services (persons, rules, procedures, payroll, etc.)
- Assist the individuals and families on understanding the process of obtaining a criminal background for staff
- Assist with understanding Integrity and Good Work Ethics for staff
- Introduce or strengten time management
- Assist with understanding part-time vs full-time staff
- Assist with understanding benefits (Social Security, Employee)
- Assist with understandingChain of Command
- Taining on reviewing cover letters and resumes,
- Assist with understanding managing staff (expectations, schedules, payroll, etc.)
- Assisting with the orientation and training process

- Assist with understanding the 12-month SDP budget
- Assist with understanding how to obtain a certified 12-month budget
- Understanding how and SDP budget is developed
- Assist with developing the SDP spending plan
- Educate on generic resources
- Understanding regional center as payee of last resort Educate on the use of SDP funds (state or federal law and regulation)
- Assist with establishing communication and coordination between the regional center, providers, and staff.
